If you already scheduled an appointment for your initial consultation with me, please locate the appropriate forms below to print. Please fill out and sign all documents to the best of your ability.

If you have any questions you may contact the office, or we can go over them at our scheduled appointment time. Please do not forget to bring a photo ID and your insurance card along with these forms on our first meeting.

You may also elect to fill these forms out in the office, in which case you should plan to arrive 10 to 15 minutes before your scheduled session time.
